That’s why Hope Senior Home Care created the Hope Transition to Home Program — a short-term service designed to make that critical return home smoother and safer.
Our team can provide transportation home, prepare your home for your arrival, stock groceries, change bed linens, tidy up, and even assist with recommended medical equipment. We’ll also review discharge instructions, help manage medications, schedule follow-up appointments, and coordinate skilled home health care if physical therapy or nursing visits are ordered.
This program helps reduce the risk of hospital readmission by ensuring all discharge plans are followed and that each person has the support they need to safely recover at home.
